Our onsite Medical House Calls follows patient-centered model of care with a personalized review of the patient's medical, psychological, social, economic, cultural, and spiritual needs being our top priority. Our health care team will provide comprehensive, continuing care all in the comfort of the patient’s home.
Our Medical Team
Our team (MD, NPs and RNs) make house calls by bringing holistic care with skilled staff and modern technology to help patients receive safe and quality care in their own homes.
Our team works closely and collaborating with patients’ previous primary care doctors, specialists, therapists, and social workers to create a successful transfer to home-based medical care. Once a patient is enrolled in the program, our skilled team can help to manage the patient acute and chronic illnesses.
